Questionnaire Assessment of Relief Risk Management in Health Services - AGRASS is an instrument that aims to collect data to describe the degree of implementation of health care risk management practices to aid decision making for continuous quality improvement care and patient safety.Specific objectives of applying AGRASS Questionnaire:1. Assess a health service as the implementation of healthcare risk management practices to guide the planning of improvement interventions.2. Re-evaluate a health service, to examine whether there was an improvement after performing interventions to improve the implementation of healthcare risk management practices.3. Compare individual health services or aggregates (eg. Health region, county, state) in the level of implementation of health care risk management practices.Target Audience of AGRASS Questionnaire:The instrument can be used by external evaluators (sanitary inspectors, certifiers, accreditors, auditors, etc.) or self-assessment initiatives of the health service. While it may be used to assist the inspection of existing health conditions, the questionnaire is not intended to serve as full script inspection, because its purpose is limited to guide the assessment of risk management assistance.Validity of AGRASS Questionnaire:The instrument is in the validation process. After literature review care risk management practices, we performed a cognitive pretest with potential customers AGRASS Questionnaire, represented by technicians from the health surveillance and health care professionals with healthcare risk management function. After changes and eliminations for specific items of the initial version of the questionnaire, the content validity and viability of all items from the current version was approved by the experts consulted. The validation process will be completed after the analysis of the questionnaire in a sample of hospitals.
